COSMETIC CORNER Facial Artistry Gregory J. Vipond MD, Inc. For this Cosmetic Corner, Dr. Vipond will be answering some additional questions that were asked of him for the iTunes Podcast called “Nothing Off Limits” hosted by Michelle Ann Owens. While the Cosmetic Corner will answer just a few of the questions, we encourage readers to listen to it, free of charge. The link to it is: “Nothing Off Limits” with Michelle Ann Owens featuring Dr. Vipond Q: Why is it important to know the differences of each surgeon? A: While there are many excellent physicians in each category, the area of expertise may differ. It is useful to match up your area of concern with the specialty of the physician. Q: When meeting a new surgeon, what questions should you ask? A: Paper credentials, such as training and board certification are a good start. While board certification is not a requirement to practice medicine, it does show a commitment towards demonstrating knowledge and competency in one’s specialty. It is important to notice if your surgeon is listening to your concerns or simply telling you what they see. Asking to see before and after photos and speaking with previous patients can help give you a feeling for their work and patient care. Q: What trends are happening right now? A: Minimally-invasive treatments, such as filler, Botox, and skin rejuvenation are popular with all of our patients. However, there has definitely been an increase in younger patients looking to prevent or slow down signs of aging. Q: Are there any permanent solutions to fillers? A: Yes, there are permanent fillers, such as Bellafill (Poly methyl methacrylate) and Silicone. However, permanent fillers are truly permanent and we cannot say what might happen years down the road. I definitely have patients who have problems from permanent fillers done more than 10 years ago who initially looked very good. My preference is to use non-permanent fillers because they will go away with time if there is a problem, but are relatively easy to repeat if they look good. Q: How can someone determine whether they need surgery, Botox, or fillers? A: Although this is a gross generalization, if you find yourself looking in the mirror and pulling back on your face, then you are simulating a face or neck lift. If you just do not like expression lines or wrinkles, then Botox may be the answer. Lines or hollows that are present without any expression may be best improved with fillers. Q: What are the different (types of cosmetic) surgeons? A: There are various types of surgeons and, unfortunately, with the change in health care, more are entering the world of cosmetic treatments. Some of the categories include: a) General Plastic Surgeons. These surgeons are generally board certified by the American Board of Plastic Surgery. These surgeons have undergone training in breast surgery, hand surgery, reconstructive surgery, craniofacial surgery, and burn surgery, although they may choose to specialize in certain areas. b) Facial Plastic Surgeons. These surgeons have come from a background in either Otolaryngology or Plastic Surgery and have undergone additional training in Facial Plastic and Reconstructive Surgery and are generally board certified by the American Board of Facial Plastic and Reconstructive Surgery. Their scope of practice tends to focus on procedures above the clavicle. c) Cosmetic Surgeons. This is a less-defined field that encompasses any physician that performs any cosmetic or appearance-altering procedure. Frequently, these physicians have come from a background of Oral Surgery, Dermatology, and Gynecology. They may be certified by the American Board of Cosmetic Surgery, although this board is not recognized by the California Medical Board.
